This review recommends that: a) ASA may be safely discontinued perioperatively when it is being given for the primary prevention of cardiovascular disease; b) patients with coronary or carotid artery stents should continue to receive ASA perioperatively; c) there is no indication to initiate ASA in patients with stable ischemic heart disease perioperatively as risks outweigh the benefits; d) patients with a previous myocardial infarction or ischemic stroke on ASA for secondary prevention, ASA in most surgeries is well tolerated, but further study is required. Comment: This review suggests that ASA be continued perioperatively in people with stents (coronary or carotid), but its use be individualized in all other patients.
