This observation study of 29 685 women who had been on bisphosphonate therapy for 5 years identified 507 incident hip fractures (1.7%). Continuing therapy for an additional 2 to 5 years did not lower fracture risk.

Comment: This study suggests no benefit, and possible harm, of continuing treatment for osteoporosis with bisphosphonates beyond 5 years of treatment.
