Pioglitazone in type 2 diabetics is associated with a 1.2 increased risk of bladder cancer. The absolute risk of bladder cancer in this study of type 2 diabetics was 0.9 per 1000 patient years. With at least 2 years of pioglitazone use, the risk increased to 1.2 per 1000 patient years. The number needed to harm was approximately 750 for a cumulative pioglitazone use of 28,000 mg and 1100 for a cumulative pioglitazone use duration of more than 24 months. Rosiglitazone does not appear to be associated with an increased risk of bladder cancer.
